When Medicare and NJ FamilyCare Overlap in Union City

Dual eligible plans in Union City, NJ are built for people who qualify for both Medicare and Medicaid. Having two programs can mean two sets of cards, two rule books and confusing bills. Special plans for dual members, called D-SNPs, try to merge the two into one experience.

We explain whether you qualify, how these plans differ from regular coverage and which of the plans we offer may fit your needs.

Who Counts as Dual Eligible

Dual status means Medicare plus full Medicaid, or for a Medicare Savings Program that helps with your Medicare costs. In New Jersey, Medicaid is called NJ FamilyCare.

Eligibility is based on income, assets and other rules set by the state. If you are not sure where you stand, we can point you to the right office to check.

What a D-SNP Does

Under the hood, each D-SNP is a specialized Part C plan. It covers your Medicare hospital and medical care and almost always includes Part D drug coverage. It also works with Medicaid so the programs pay in the right order.

  • Many plans include a care coordinator who helps arrange visits and services
  • Most offer routine dental, vision or hearing benefits
  • Some include allowances for over-the-counter items or rides to appointments
  • Costs are often very low because Medicaid helps cover your share

Extra Chances to Change Plans

People with both Medicare and Medicaid have more flexibility than most. Federal rules give dual eligible people special chances during the year to change certain plans, not just in the fall.

That flexibility is helpful, but switching too often can disrupt care. We look at your doctors, medicines and home care needs before suggesting any change.

Keep Your Medicaid Active

Your D-SNP depends on your Medicaid. If your Medicaid renewal is missed, you could lose the plan. Watch your mail for renewal forms from NJ FamilyCare and return them on time.

If paperwork is hard to manage, ask a family member to help, or call us. We can explain what the letter is asking for in plain Spanish or English.

Help Beyond the Plan

Union City's Senior Citizens' Affairs office offers transportation to doctors and shopping for eligible residents. Pairing that local help with a well-coordinated plan can make daily life easier. If you do not have full Medicaid, see help paying for Medicare for other programs.

What Union City Asks About Dual Plans

Will I pay a premium for a D-SNP?

Many dual eligible members pay little or nothing, because Medicaid helps with costs. It depends on your level of Medicaid.

Can I keep my current doctors?

Only when they belong to that plan's provider list. We check each doctor before you choose.

What if I have Medicare and only a savings program?

You may still qualify for some D-SNPs. We can check which of the plans we offer accept your situation.

Does a D-SNP replace my Medicaid card?

Not always. You may still use your Medicaid card for some services. We explain which card to show and when.
